Chronic Disease Management [CD]

Requirements

[CD1]: A monitoring solution for patients to prevent re-ulceration on their feet

Some diabetic patients may not see the need to attend podiatry appointments to check their feet once their ulcers have healed. Defaulting subsequent podiatry appointments can result in the thickening of skin lesions at the previous ulceration site, resulting in re-ulceration. NHG is seeking a home monitoring solution for diabetic patients that can predict and help to prevent cases of re-ulceration.

[CD2]: Making digital home monitoring of eye conditions easy for all

Prevalence of many eye diseases increase with age. There are new models of care that allows stable patients with no or minimal diabetic retinopathy to be monitored annually via digital photography at polyclinics instead of SOCs. To push eye care further beyond hospital to the community, the greatest challenge will be for all stable Ophthalmology patients to be able to do these fundus photos at home by themselves or their caregivers, without pupil dilation, at low cost, without costly bulky equipment. Patients can then transmit these images to link up with existing image reading setup, benefitting from current image reading protocols and future AI developments.

[CD3]: Customised footwear for diabetic patients or patients with foot conditions/complications

NHG is seeking commercial partners who can produce customisable specialist footwear for patients with diabetes, or people with foot deformities or conditions. These footwear should preferably be easily personalisable, adjustable for prescription, functional for this specific patient population, aesthetically pleasing, and affordable.

[CD4]: Specialist footwear that is functional and aesthetically pleasing

Persons with various foot conditions often need specialised footwear that have more internal volume to accommodate food orthoses and total contact insoles. Currently such footwear tends to be chunky, non-aesthetic and costly. The TTSH Podiatrist team is looking for companies that can provide specialist footwear without the current shortcomings.

[CD5]: Diabetic foot screening, stratification and predictive analysis

The NHG Polyclinics team is looking to develop an integrative on-site diabetic foot screening prototype that can screen, risk stratify and perform predictive analysis for large-scale foot screening to prevent lower limb amputations. This prototype can achieve comparable results with standard diabetic foot screening currently carried out by trained nurses.
